Once the addressee of your report has been identified, the rest is usually easy. Spam reports can be written in a standard manner. All it really needs to contain is the full source of the unsolicited email you received and a grain of politeness. It is also best to keep the message brief. Abuse departments get a lot of mail.
For example, a spam report about our example spam could read:
Hi,
I received the spam included below. It has been sent using your resources or makes it look as if it originated with you. Please investigate and take the appropriate actions to stop this abuse.

Typically, you will not get a response to your spam report. This does not mean that the ISP has not taken action, however. You will not usually be the the only person who got spammed from the same source, and you will not be the only one reporting the spam either. Responding to all reports is often difficult.
